$100.00 includes the following:

• Visit with doctor, T.B. skin test (if ordered by doctor)
• Blood draw for any blood test ordered by the doctor.
  
  Note: All blood work is sent to Quest Diagnostics. You will receive a bill from them for any blood work that is ordered by your doctor.
  
  
• You are required to return to our office in 48 hours to have T.B. skin test read by lab tech.
• If T.B. skin test is positive a chest x-ray is required at the cost of $40.00
• If any immunization is needed you will be sent to the Health Department at your own expense.
• After you complete the above you will need to return to the clinic with copies of all results and shot records.
• Paperwork will be complete and given to you in a sealed envelope.
